2013-2014 District Governor Team Excellence Award Worksheet

The Team Excellence Award recognizes district team members who achieve membership growth and excellence in district leadership. To be considered, complete this form and mail it to the English Language Department at international Headquarters, or fax it to 630-706-9130. The application must be received before September 30, 2014.  The award will be sent to the 2014-15 District Governor for presentation. REQUIRED ACHIEVEMENTS: All five requirements must be met to qualify: SERVICE - Ninety percent (90%) of the clubs participate in a meaningful and visible service project.  When 1VDG or 2VDG visit a club report to DG Ron Mitchell the meaningful and visible service project for the club you visited. MEMBERSHIP - The district had a net increase or chartered a new club. Net Membership Increase: Name of Newly Chartered Club: Tillman’s Corner Lions Club LEADERSHIP DEVELOPMENT - The district provided training and mentoring for club officers. Date of Training: July 27, 2013 at the Summer Conference/Cabinet Meeting Number of Participants: 30 Description of training: Club President Training, Club Secretary Training, Club Treasurer Training CLUB DEVELOPMENT - Ninety percent (90%) or more of the clubs are active and in good standing. CS Yvonne Burroughs monitor and report. ADDITIONAL ACHIEVEMENTS: In addition to the required achievements noted above, the district must also complete ten of the following achievements: 1. District team hosted a new member orientation. Fall Conference by DG Ron Mitchell 2. District team hosted a Certified Guiding Lion training. Winter Conference 3. District hosted an Extension Workshop or planned an extension initiative. Sunday, September 29, 2013.  Instructor PCC Carl Harrell 4. District hosted the Club Excellence Process Workshop. GMT Mary Turner, November 16, 2013 at the District Fall Conference. 5. District gained new members by initiating a successful district wide membership campaign. Membership campaign conducted by GMT Mary Turner 6. 90% of the clubs submitted a Membership Report by the end of the year. Monitored by CS Yvonne Burroughs. 7. 75% of the clubs in the district submitted an activity report.  Monitored by CS Yvonne Burroughs 8. 100% of the clubs have submitted the Club Officer Report (PU-101) before the close of the year.  Completed by CS Yvonne Burroughs 9. A number equal to 10% of the clubs in the district were awarded MJFs or progressive MJFs. PDG Dec McCelland will monitor and report. 10. 50% of the clubs supported LCIF. PDG Dec McCelland will monitor and report.
